AUTHORIZATION TERMS

Ten important topics.

All ten topics are shown below. You can collapse or reopen any section.

Before you sign: Apex staff must first explain the specific proposed repairs, estimated charges, and anticipated completion date. This form does not claim you received or reviewed an estimate.
01Scope of work and repair plan

Before I sign, Apex staff must explain the specific repairs proposed for my vehicle, the estimated charges, and the anticipated completion date. By signing, I authorize only the work described to me by Apex for this repair. This form does not represent that I received or reviewed an estimate, and it does not authorize unspecified repairs or open-ended charges.

02Inspection, disassembly, and reassembly

Inspection, scanning, teardown, or disassembly is authorized only when Apex has described that work and its charges to me. Finding hidden damage does not authorize additional repairs. Any reassembly limitations and charges must be explained before disassembly; separate approval is required before additional work.

03Scans, diagnostics, and vehicle data

Diagnostic activity is limited to what is reasonably needed for the work Apex explained. Access to event data, personal communications, or unrelated vehicle information is not included and requires separate explanation and authorization.

04Insurance coordination and payment

Apex may discuss repair documentation with an insurer as appropriate. The insurer makes its own coverage and payment decisions. This authorization does not direct insurance payments, assign proceeds, or give Apex authority to endorse insurance checks. Any customer payment responsibility will be explained separately before work begins.

05Hidden damage and supplements

Hidden damage may require a change to the repair plan. This signature does not approve additional or changed repairs in advance. Apex must explain proposed additional work and estimated charges and obtain my separate approval before performing it.

06Parts, refinishing, calibration, and fuel

Parts and operations are limited to the specific work Apex explained. Any optional parts upgrade, added fuel, refinishing material, diagnostic operation, or calibration outside that work must be explained with applicable charges and separately approved.

07Vehicle movement, test drives, and sublet work

Apex employees may move or road-test the vehicle as reasonably necessary to perform or verify the specific authorized work. A nonroutine transfer to another repair location or unrelated work requires separate explanation and approval. Nothing in this authorization waives responsibility for negligence.

08Pre-existing damage, belongings, and parts notice

Apex may document visible pre-existing vehicle condition. Please remove personal valuables before repair. You may ask Apex about inspecting replaced or repaired parts, subject to applicable exceptions. This authorization does not waive statutory consumer rights.

09Storage, payment, and vehicle release

This form does not set storage rates, a storage start date, or other shop charges. Apex will disclose applicable charges and timing separately before they apply. Payment and vehicle release follow the applicable shop records and law; this authorization does not add undisclosed fees or blanket responsibility for insurer-denied charges.

10Electronic signature, communications, and copy

I agree to sign this authorization electronically. My single signature applies to the specific work Apex explained and the applicable terms above; it does not authorize unspecified repairs. Apex will record the signature date and time automatically and email the completed signed PDF to the customer email provided and to mail@apexcollisionrepair.com. Repair messages are separate from marketing.
